This sounds like my cousin. DRE completely normal, and he had had a UTI, after two weeks and after two months of ciprofloxazin, still PSA of 10 with a free fraction of 0,16, I decided to biopsy.
I don't remember his Gleason score now, but it was a mainly very well differentiated adenocarcinoma. He underwent RPE.
I feel that a small prostate in a 41 year old where infection has been treated, should not have a PSA of 10.
