Well, CS all of your patients of this kind seem to develop cancer unless active surgery is performed. In this case it seems a truly bad idea to leave them in situ. There might at least already be CIS in them, and the guy is probably azoospermic. What about other tumor markers? The question seems more like how cautious to be about the possibly already existing malignancy.
Are we to expect, as usual, that you already have the results at hand?
