Some of these patients are very prone to constipation but there is very little published on their anorectal physiology. The Ameriacan of course would treat him with ISC and alphablockers. He has impaired kidneys already at this age and the risk of ultimate end stage failure is quite high. He has urethral sensation and would not cooperate with ISC, particularly after a traumatic experience with our houseman catheterising him urethrally. I did not not think I could trust him to comply with urethral ISC.

After discussing with Tim Stephenson, I carried out a "clam" sigmoid cystoplasty and Monti's procedure for him. The Monti with allow him to catheterise painlessly.
