In the early 80s, the standard teaching for this (T0-focal CaP as it was called then)was to do a reTURP to and get all the chips examined to see if there is any more cancer. I still believe there is a case for this in this man if the tumour is truly unifocal (i.e. if the chips have been adequately sampled - therefore the relevance of size of resection and percentage of chips submitted for block). Absence of tumour on reTURP would give me the confidence to watchfully wait and he should be adequatedly reassured. If there is tumour on ReTURP, I would advise radical prostatectomy after 3 months.
