There is very little literature on this age group of patients. When we reviewed our experience at WGH, 60 used to be the conventional cutoff point for young CaP. I think it still is. There was great interest in young CaP in the 80's until a paper was published which more or less resolved the issue (I have to dig it up). The believe then was that young CaP do not do as well. When the blacks were taken out, the survival statistics were the same for young whites and age was not a significant variable. Differentiation of tumour was significant.
I have heard this argument many times - young CaP do them whatever their age i.e. if they have more than 20 years life expectancy. Again there is no evidence even though I am sympathetic towards this cause and will discuss the option very seriously with a patient. The truth is as always, if you do enough of them, you cure some of them. Our biopsy predictability in the relevant group of patients is at best 70%.
Is there a correlation between gleason score and PSA values? Any references?
