My late chief of urology - Professor Chisholm - maintains that unless one can separate the pussycats from the tigers, there is no evidence that RRP has made any difference in the survival of patients with CaP. My impression is that in the group of patients where surgery actually makes a difference, is the group with a very narrow window of opportunity. Unless we can open up this window through more precise early diagnosis of this group of patients, we will have to take the apparent shame of facing failures while saving some of them. Even with this attitude, there are so few of such patients in my practice. PSA, being what it is in our local setting should not be too restrictive if bone scan is negative (when indicated), and TRUS shows no capsular invasion, and MRI (if you do it) shows no seminal vesicle involvment.
