He has a delayed wound infection, or a delayed manifestation of a wound infection - something we have been seeing in patients with contaminated wounds who have received powerful antibiotics during surgery. If your urethrogram does not show any stricture distally, put a spc and wait. it should heal up satisfactorily, although it is going to take weeks.
