59-year-old Malay man with well controlled NIDDM. Fractured pelvis with ruptured membranous urethra managed initially with suprapubic drainage alone as he was initially very sick in intensive care. Up-and-downogram showed a 1 cm distraction which was easily bridged by urethral mobilisation and corporal separation alone and when I repaired it 3 months later. The anastomosis was tension free. Initial outcome was satisfactory but 3 weeks later, the perineal wound started to discharge pus and later urine. Urethrogram 4 weeks after repaired showed a leak from his bulbar urethra! What is going on? What do you do now?
