I agree. Even if the nodes are negative, the likelihood of capsular involvement is rather high. I think I would probably go for DXT using a conformal protocol. I would probably start with androgen withdrawal first followed by DXT or if the patients wishes to remain sexually active, DXT first and add hormonal therapy if PSA nadir is above 0.5 after DXT.
