In Sweden this patient (provided TRUL does not reveal capsular engagement) would be offered diagnostic lymfadenectomy, and if negative most likely a radical prostatectomy. Otherwise external radiotherapy (depending on domicile in the country, i.e. depending on what hospital would manage the patient, with or without lymfadenectomy, or the patient would in the two centers ursing it, be offered interstitial + external radiotherapy). That is how strict our principles are, in spite of having a far larger number of cases oc CaP...
