As second visit surgery is going to be major, which ever approach you fancy, I would ideally like to justify it with histological confirmation of recurrence. The trouble is, we are talking about a localised thicking of the right crus of the diaphragm which was not there on the previous CT. sitting right next to the aorta, making biopsy hazardous, hence the wait. Besides, 18 months is a fairly long time for what appear to be a relatively small lesion.
