Cont. from PSA article posted by CDDing 1/31/98: The followings are a dozen examples, some deliberately controversial. Feel free to argue each case:
PSA indicated?:
Asymptomatic men >50 with brother or father died of Ca P.
Small "fibrous prostate", any age, in whom metastatic adenoCa demonstrated at a distant site, eg. liver, SC node.
Men with life expectancy > 10 yr, PSA >2.7 but <4 done 6 months ago.
Fairly fit men <70, LUTS with benign DRE.
Any man for TUVP, ELAP or other procedures which would not yeild tissue for histology.
Somebody whose PSA 2 months ago was 10, 1 month ago (after antibiotics) was 4.2.
Very fit 75 yr-old, PSA was 5 a yr ago, 4.5 six months ago.
72 yr old, PSA 6.5, DRE: 80g+ BPH, prostatectomy done, histol BPH.
45 yr old, LUTS, flat benign feeling prostate, responding to alpha blockers.
55 yr old LUTS, asymetrical but soft smooth prostate.
60 yrs old with LUTS, on Proscar 1 yr. PSA done 6 months after treatment = 3.0.
Any fit patients <70 with unremarkable DRE to be considered for alpha blockers or Proscar.
