I have been trying to put together the information on operable Ca Prostate - whether there is anyone really worth operating on!! My hypothesis is that when we have excluded those group of patients who will not benefit from radical prostatectomy, the percentage of patients with CaP that is worth operating upon is really very small.
A few things we do know: i) PSA >30 chances of boney mets is high, if not at time of diagnosis, it will present soon after;
ii) Gleason 7 and above, margin Positivity and seminal vesicle invasion >30%, similar percentage for lymph node involvement;
iii) PSA <10, Gleason 3 and below, most likely are pussy cats needing no help from us!
iv) neither Gleason score nor initial PSA predicts outcome in all patients;
v) when we transpose Partin's indices there really are very few malignant prostates worth operating on.
