: 57 years old man. Posterior urethroplasty done for fracture pelvis with urethral
rupture, 10 years ago. Now presents with LUTS, mainly obstructive in nature. IPSS 15,
Bothersome 3. DRE showed BPE. USS shows normal upper tracts and PVR of 50 cc.
Cystourethroscopy showed no urethral stricture, a poorly defined veru, succulent looking
lateral lobes of the prostate. Which way to go next.
I would put the patient on oral medications as he has moderate symptoms. Should this be unsatifactory then a VCMG should be done prior to surgical intervention. I would opt for alternative therapy like TUNA as it would have little to no risk of bladder neck damage and get rid of the lateral lobes.
