I choose to disagree with Chek Lang on this one (& hope that he doesn't get too violent!). While I agree that the bladder should come out irrespective of CMG findings (given that histologically, this was definitely a TCC), a CMG showing a neuropathic picture would alert one to look at this guy's spinal cord and also make me more willing to accept 2 separate pathology here, ie, a neuropathic baldder with an incidental superficial tumour. If CMG shows just hypocompliance and sensory urgency, I would look harder for diffuse CIS. Clinical neurological examination is too blunt an instrument to look for an occult cord lesion although you can argue that you should just proceed to an MRI scan without CMG. If the diagnosis of TCC is questioned, of course CMG would be even more important as you are likely to reconstruct on this baldder!
