If the diagnosis of TCC is not in question (catheter cystitis/granuloma can occasionally
to mistaken even histologically as TCC), there are 2 separate pathologies: TCC and high
pressure chronic retention or even neuropathic bladder.
As far as the upper tract is concerned, this is a dangerous bladder. What to do about
it depends on how certain the diagnosis of TCC is. If diagnosis is certain, eventhough low
grade, I would not reconstruct on this bladder. What are the urodynamic parameters. Was
the U/S and antegrade done with CBD? How is the serum creatinine now with bilat. JJ stents
(I assume he is voiding.)
