5 cm is the cut off size for adenoma and adenocarcinoma of the adrenal gland and any
adrenal tumour removed which is above 5 cm should be followed up closely even if the HPE
shows it to be benign. An incidentaloma >2cm is considered significant and needs to be
removed. I will proceed to do a right nephrectomy and leave behind the right adrenal gland
- should be easy unless directly infiltrated by the RCC (unusual in my experience).
Suggest you go for the upper pole of the kidney after having lighted the vessels. Then
leave the adrenal behind and the rest of the operation to continue as for radical
nephrectomy. If HPE confirms negative pedicle nodes, I will get back in a couple of months
to take out the left adrenal gland. If her pedicle nodes are involved I will just watch
her from then on.
